It is a requirement of our rules and constitution that all fixtures are firstly made by telephone and then confirmed, on one of these slips, in writing.

You should fill in two identically, send one of them to your opponent and keep the other for proof in any later dispute.

ALL clubs must use these slips. More, if required, can be printed from here or the Secretary will, if asked, supply them.

Remember to enter all confirmed fixtures on Secretary's list plus your own copy of same.
